Mercy Medical Center is partnering with all Baltimore City hospitals and the Baltimore City Health Department to collect community health information for its Community Health Needs Assessment (CHNA). Your responses to this optional survey are anonymous and will inform how hospitals and agencies work to improve health in our Baltimore community. You must be 18 years or older to complete this survey.

Community Health Needs Assessments are required of tax-exempt hospitals as a result of the Patient Protection and Affordable Care Act. The CHNA and implementation strategies create an important opportunity to improve the health of communities by ensuring that hospitals have the information they need to provide community benefits that meet the needs of their communities. They also provide an opportunity to improve coordination of hospital community benefits with other efforts to improve community health.
